I fully understand that LOGOS is a Christian School, that my child will be taught Bible, attend chapel programs and that children/parents of other faiths and persuasions will not be allowed to proselytize.
I agree to abide by the rules and regulations of LOGOS and the jurisdiction of the laws of Cambodia.
I understand that LOGOS will not be held responsible for any accidents or injuries occurring to my child(ren) while at school, on school-sponsored trips or while being transported by the school.
I understand that students with outstanding accounts may be refused entry into class or the following semester until the account is settled.
I understand that if I have given false or inaccurate information on this application, my child may be dismissed from the school.
I understand that the personal information provided will be used for school administration and academic purposes and may be shared with Asian Hope management.
I understand that my child's photo or work may be used in school publications, the website, or social media unless I notify the office in writing otherwise.
